Please fill out the form below as completely as possible. All submissions will be kept in the strictest confidence and nothing will be used without written permission, however, all talent will be required to sign a model release at the time of the shoot. All videos require you to show your face, so please be forewarned. Think this through prior to submitting this form, as cases of "cold feet" are a waste time for all parties involved. Videos will primarily be shot in the Northern California Bay Area, particularly in the areas in and around Santa Clara County. Shoots will be on the weekends only. You will be responsible for all travel and accommodation expenses. Shoots will typically last one day, however, if multiple day shoots are required you will be informed in advance. All those who wish to apply
to appear in a video must fax this form (just hit ctrl+P after filling
it out) plus a recent photo to (408) 985.1450, On the day of the shoot you will be required to bring the following. If any of the following are missing, you may (no guarantees) be asked to come back at a later date with the missing information, but the day's shoot will not be allowed to continue. In the event of this unfortunate happening, all expenses you incur during your trip will not be covered. SO MAKE SURE YOU BRING EVERYTHING!: -Recent PCR/DNA test (AIDS test) results from Aim-med. (2 copies, one will not be returned to you.) You can be tested at a local draw station. To find your nearest draw station location call (818) 981.5681 Monday through Friday from 10am-5:30pm PST. and Saturday from 1pm-5pm PST or visit www.aim-med.org for more information. Tests administered by your physician will not be accepted. We will ONLY accept tests administered by AIM. -2 forms of ID, one with photo (Driver's license or passport) SERIOUS INQUIRIES ONLY, PLEASE! |
